A woman drowned in a lake near her home following a catalogue of failings by mental health services, a coroner has found.
Julie Carter-Montacute was discovered by police divers in Chew Valley Lake on February 22, 2018, after going missing for the third time in a week.
The much loved mum-of-two struggled with depression for a number of years, and was sectioned in May 2017 after attempting to take her own life.
